Q: Do I need a TEFL certificate applying for teaching jobs?
Do most teaching jobs require a TEFL or not? Or just a BA degree will be sufficient for work visa?

No, you need BA, TEFL Certificate and 2-years working experience (which can be in non-teaching).

Working (Z visa) for teaching English
1. BA Degree or Higher (Essential)
2. 60/120 hour TEFL certificate (Or TESOL, CELTA etc. Can be completed in 2 days online. click here)
3. Native English Nationality (UK, USA, Canada, Australia, Ireland, New Zealand or South Africa)
4. 21 to 55 years of age
5. 2 years post-graduate work experience (In any field, ONLY for Beijing, Guangzhou, Shenzhen, Shanghai - Other cities DO NOT require this)
Note: If your BA is in an education related field a TEFL is NOT required, 120 hour TEFL required for Shenzhen, Beijing, Guangzhou and Shanghai

Do you have any other sources/insight on point number 5? I have seen some Tier 2/3 cities requiring two years of work experience, but it seems very random in how strictly it is implemented

Requirement for 2-years of working experience is there to prevent too young people applying for teaching job in China. In other words, if you are 28-years old without 2-years of working experience, they'll accept it. I guess! It is personal experience, so it is difficult to say how it is going to end up in your case.
You can search Yahoo-gle for more sources on 'Z visa (WP) requirements in China', but yeah in China that requirements depend on SAFEA, i.e. 'will Gov. in your city/Province accept your CV and docs. displayed'.
I am Non-native English and I worked all around China, in smaller cities mostly. I even had WP in Dalian, Liaoning, where ''NES' was requirement for WP" since I know (2009).

To put it more to the point: You don't need it if you have proof of two years of experience in the related field. That could be a company recommendation letter, get it translated into Chinese if its in English.
It totally depends on your school requirement ,and your background ,not really necessary if you are a native speaker
