I'm dogsitting for a friend and realised I have no proof to show that he's been registered and gotten all the necessary vaccinations. What's the worst thing that can happen if someone checks?

The worst thing that would happen would be that the person that checked will tell you to get it vaccinated. Now if the dog were to bite somebody and then they found out you didn't have proper papers that's a whole different story....
Since you are kind enough to help your friend dog-sitting, you should not worry about this, but when you friend returns to Shanghai you can pass on this information to her:

The dog is far more at "risk" than you and your friend in the event of an outbreak of an animal virus resulting in a government campaign against pets without vaccination and registration papers. This did happen a few years ago.

Actually, it depends on where you are. Sometimes there are crackdowns on non-registered dogs. In Shanghai during a crackdown, the rule is that if you don't have your dog registered, they will simply take it and put it on a truck bound for Guangzhou. ALWAYS register a dog in China and carry the papers with you when you go out for walkies.
