If you are teaching English then you'll need to be from USA, UK, Ireland, Canada, Australia, New Zealand or possibly South Africa. Your employer should cover the cost of your Z visa.

The fee at the PSB office is 400 rmb, training schools get 6 months, government schools get a year in most cases.
I am not sure of the fees for card and agency that certifies your documents because you are not required to appear there and the school takes care of this. Incidental costs, some PSB offices want a digital photo that can be put in facial recognition and charge you for this at the office. Many provinces will not take your physical from abroad even if completed on the correct Chinese form and will make you take another physical in China. Universities usually have their own clinic do this for nothing but training schools may deduct this from your first paycheck.
